How to use this service:
Show your ID card to the provider’s assistant each time you visit the
provider.
To access the Chiropractic network,
view the database of chiropractors
online or
call the number on your ID card and mention that you are a UCA member The customer service representative
will request your name, group number, membership number and zip code.
Once your information is confirmed, you will be provided with the names
and telephone numbers of participating chiropractors in your community.
Identify yourself as a member of Chiropractic Lifecare of America*
network when making an appointment
with a participating chiropractor.
*Note: This program is NOT insurance. It is a reduced-fee-for-service
program that provides discounts at certain providers. In order to
receive the special pricing, you must pay in full at the time of service
and verify that the provider is an active participant of our
chiropractic network.
